Application progress of online grief counseling in palliative care

Grief counseling is one of the main contents of palliative care,which plays an important role in alleviating the grief of terminal patients and bereaved people. As a new platform for grief counseling,the internet has the advantages of cross-time space,anonymity,flexibility and so on. This paper reviews the relevant theoretical models,application carriers,intervention methods,feasibility and effectiveness evaluation tools or indicators of online grief counseling,analyzes the dilemma of using internet to carry out grief counseling,and puts forward clinical practice suggestions,aiming to provide references for domestic medical staff to carry out relevant research and promote the application of online grief counseling in the field of palliative care.

Development and evaluation of a risk predictive model for high level of self-reported symptom cluster distress in breast cancer patients undergoing chemotherapy

Objective To develop and evaluate a risk predictive model for high level of self-reported symptom cluster distress in breast cancer patients undergoing chemotherapy. Methods 647 patients who received chemotherapy and met the inclusion and exclusion criteria were selected at 4 tertiary hospitals in Shanghai,Shandong,Jiangsu,and Zhejiang from October 2019 to May 2021. Cases were randomly assigned to a modeling group and a validation group based on 5-fold cross-validation method at a ratio of 8:2. The random forest algorithm was used to develop the risk predictive model in the modeling group. The receiver operating characteristic curve,Hosmer-Lemeshow goodness-of-fit test,calibration curve,and decision curve were used to comprehensively evaluate the prediction performance of the model in the validation group. Risk factors were identified based on the order of the importance of each influencing factors. Results The incidence of high symptom cluster distress was 33.27% in the modeling group and 29.23% in the validation group. The area under the receiver operating characteristic curve of the prediction model was 0.91;the sensitivity was 65.8%;the specificity was 93.5%;Hosmer-Lemeshow goodness-of-fit test was insignificant(P=0.1365);the decision curve was above the reference line. Body image,self-efficacy,and financial burden were the most important predictors. Conclusion The risk predictive model based on random forest algorithm has good predictive performance,which is of great significance to help identify breast cancer subgroups at high risk of symptom cluster distress,and will potentially promote symptom management.

Video education for advance care planning:a scoping review

Objective To review the application of video education in Advance Care Planning(ACP),and identify the elements of video education for ACP,so as to provide the guidance for future research. Methods A literature search was performed in the PubMed,Embase,CINAHL,Web of Science,PsycINFO,Cochrane Library,CNKI,Wanfang,VIP and CBMdisc. The retrieval time was from the establishment of the database to April 30,2022. The included studies were summarized and analyzed. Results A total of 27 articles were included. Forms of video educational include mobile terminal devices,telemedicine. The core content includes providing ACP related knowledge,providing disease-related knowledge and information on the advantages and disadvantages of treatment measures,clarifying values,goals and medical care preferences. The outcome indicators were mainly the process indicators,action indicators,care outcome indicators,feasibility evaluation. Conclusion Video education in the field of ACP presents preliminary effects and feasibility. It is suggested that researchers could draw on and refer to foreign ACP video education tools,develop and update scientific and feasible ACP video education tools for our country,explore appropriate ACP video education strategies,improve the effectiveness evaluation index,and improve the quality of ACP video education.

Currently,the survival time of colorectal cancer patients has been gradually extended,but cancer and treatment can cause many troubles for them. Patient-reported outcomes with colorectal cancer focus on assessing the physical and psychological experience of patients,which helps to identify their health challenges and give targeted treatment. This article reviews commonly used patient-reported outcome measures in the field of colorectal cancer at home and abroad and their clinical application. The content characteristics,existing problems of the current tools were comparatively analyzed. Future research directions are proposed. It is intended to provide a reference for the application,development,and clinical practice of patient-reported outcome measures for colorectal cancer patients in China.

Expert consensus on parenteral nutrition safety infusion

Objective To form an expert consensus on parenteral nutrition safety infusion, and standardize the operation process of clinical parenteral nutrition safety infusion,so as to reduce the parenteral nutrition complications. Methods On the basis of the evidence-based method,we searched the high level evidence about the parenteral nutrition safety infusion in databases such as UpToDate,BMJ Best Practice,National Institute for Health and Care Excellence,Guidelines International Network,National Guideline Clearinghouse,Registered Nurses' Association of Ontario,Scottish Intercollegiate Guidelines Network,European Society for Clinical Nutrition and Metabolism,American Society for Parenteral and Enteral Nutrition,Chinese Society for Parenteral and Enteral Nutrition,China Guide Network,Joanna Briggs Institute,China Biomedical Literature Network,VIP,Wanfang,CNKI,PubMed,Web of Science,Scopus,Embase. The retrieval period is from the establishment of the databases to January 2021. Studies were evaluated and collected; relevant recommendations and research results were selected to form the draft of the expert consensus. Experts were invited to complete 2 rounds of Delphi expert consultations and 2 rounds of expert meetings. Each item was revised and improved through 2 rounds of Delphi expert consultations and expert meetings,and the final draft of the consensus was formed. Results The positive coefficient of experts in the 2 rounds of correspondence consultations were 100%; the expert authority coefficients were 81.00% and 89.50% respectively,and the coordination coefficients of expert opinions were 0.13 and 0.15 respectively. Finally,the consensus summarizes the latest evidence on 4 aspects of multidisciplinary team formation,evaluation,operation norms,and complication management. Conclusion The establishment of the consensus can provide a reference for the standardization of safe infusion of parenteral nutrition in clinical practice to ensure patient safety and improve service quality.

Construction of nursing-sensitive quality indicators system of enteral nutrition on the basis of evidence-based approach

Objective To construct nursing-sensitive indicators system of enteral nutrition for hospitalized adult patients,so as to provide scientific,objective and quantitative monitoring standards for nursing quality of enteral nutrition.Methods Evidence synthesis was conducted based on Johns Hopkins evidence-based nursing method for laying the foundation for nursing-sensitive quality indicators system of enteral nutrition. Through group discussion,appropriate indicators feasible to application were determined,and nursing-sensitive quality indicators,calculation formulas and data collection methods were preliminarily decided. Through two rounds of expert consultation,the nursing-sensitive quality indicators system of enteral nutrition was improved and established.Results The constructed nursing-sensitive quality indicators system of enteral nutrition included 3 first-class indicators,6 second-class indicators,and 17 third-class indicators. Positive coefficients of two rounds of expert consultation were 92% and 100%,authority coefficients were 0.915 and 0.925,respectively,and the coordination coefficients were 0.124~0.242.Conclusion The nursing-sensitive quality indicators system was scientific and practical and it was beneficial to improve the nursing quality of enteral nutrition.

Evidence summary for prevention and management of deep venous thrombosis after spinal surgery

Objective To select and obtain the best evidence for deep venous thrombosis(DVT) prophylaxis in spinal surgery patients after operation so as to provide reference for clinical practice to decrease the rate of DVT. Methods Using the method of evidence-based nursing,clinical question of DVT prevention in spinal surgery patients was raised. Relevant research was retrieved by computer search. JBI evidence appraisal and recommendation system were used to evaluate quality of studies and level of evidence. Results Combined with judgment of clinical professionals,totally 27 evidences selected,including DVT assessment,diagnosis and screening,intervention strategies,prevention time and methods,health guidance,etc. Conclusion This study summarized the best evidence for prevention and management of deep venous thrombosis after spinal surgery,and help clinical nurses and nursing admi-nistrators decision-making,ultimately through adapting the best evidence and using scientific nursing methods to solve the clinical problems and improve nursing quality.
